Available from Rüdiger Carl: carl@pol-music.de. This is an amazing production, being, in effect, two large beer mats, 1" diameter larger than the CD with the CD sandwiched between and the whole kept together by a yellow double rubber band. And wonderful artwork on the front, as indicated above. Beautiful.
